The meeting reports and assessments for use in 2012 can be found below. The Guidance notes for each role are in PDF format. The Meeting Reports and Assessment paperwork are in Word format so they can be completed either electronically or by hand. Some events will have the functionality of Online Meeting Reports and Assessments. For these meetings reporters will be sent user name and passwords to enable them to complete their reports and assessments online. The forms that have been amended in January 2012 have been labelled as such. There have been no other changes other than those labelled.
